“A refusal letter is a first draft, not a verdict.”
I did eight years in community advocacy before this and it turns out the skills transfer completely. An insurer's refusal, a pharmacy shortage, a regional clinic with a nine-month wait — these are not medical problems, they are administrative ones, and administrative problems have processes. Most people lose to them because nobody told them there was a process.
So insurance-appeals is unromantic and extremely practical: what the refusal letter actually said versus what it appeared to say, which criterion you are being measured against, what evidence answers that criterion, how to write four paragraphs that a reviewer can act on. We have a running count of appeals members have won and it is one of my favourite pages on this site, not because of the number but because every one of them was somebody sitting down at a kitchen table and doing a boring thing well.
Rural-access is harder and I will not pretend otherwise. Sometimes the honest answer is that the nearest prescriber is four hours away and there is no clever fix. What we can do is share what has worked — telehealth that actually takes new patients, pharmacies that will transfer, the wording that gets a referral moving — and be company for each other in the meantime.

Cost is a clinical variable, not a character test — how members have planned for it, which levers are legitimate, which are dangerous, and how to stop well if it becomes untenable.
